I have not been able to get this camera to connect via rtsp, I've tried the following and more...
rtsp://192.168.1.15/ch0_0.h264
rtsp://192.168.1.16:554/
rtsp://192.168.1.16:554/?user=admin&pwd=admin
rtsp://192.168.1.16:554/video.mp4
Any other suggestions?

did you try logging into the camera, it will usually tell you where the feeds are.

Just a thought, if you are still trying to figure this one out.
It appears that Lorex uses two different passwords. Which of the two is used for your camera does not appear to be well documented. The two that I know of now is "000000" and "admin". From your configuration listed above it appears that you used the latter. Try using the former of "000000".
